ADDENDUM TO ADMINISTRATIVE SERVICES AGREEMENT <br />THIS ADDENDUM TO ADMINISTRATIVE SERVICES AGREEMENT, (this "Addendum"), entered <br />into effective as of October 1, 2019 (the "Addendum Effective Date"), is made by and between RxBenefits, Inc. f/k/a <br />Prescription Benefits, Inc. ("Administrator"), and Indian River County Board of County Commissioners <br />("Client"). The parties, intending to be legally bound, hereby agree as follows: <br />1. Administrator and Client are parties to that certain Administrative Services Agreement <br />dated May 1, 2018 (the "Agreement'). <br />2. Administrator and Client hereby execute this Addendum for the purpose of documenting <br />that Exhibit A (Client Application) to the Agreement has been amended and restated to reflect, among other <br />things, new pricing terms. Such amended and restated Exhibit A (Client Application) shall be attached and <br />affixed to the Agreement as Exhibit A (Client Application) in lieu of the prior Exhibit A (Client Application) <br />upon execution of this Addendum by the parties' authorized representatives below and shall be in full force <br />and effect as said Exhibit A from and after the Addendum Effective Date. <br />3.
