Brown, County Administrator <br />From: Vincent Burke, PE, Director of Utility Services <br />Prepared By: Arjuna Weragoda, PE, Capital Projects Manager <br />Subject: Amendment No. 1 to Work Order No. 3 with Kimley Horn and Associates for Floridan Well <br />Deepening and Performance Improvement at South County RO Plant <br />Descriptions and Conditions: <br />On July 12, 2016, the Indian River County Board of County Commissioners (BCC) awarded the contract for <br />the South County Reverse Osmosis (SCRO) Water Treatment Plant (WTP) Well Number 7, Wellhead and <br />Appurtenances, and Floridan Aquifer Wells Rehab Project to All Webbs Enterprises, Inc. (AWE), at a cost <br />of $1,477,360.00. <br />During the initial phases of rehabilitation of well 5-4, it became apparent that the existing polymerizing <br />vinyl chloride (PVC) casing was damaged significantly to the extent of not being salvageable. Therefore, a <br />change orderwas issued to AWE to drill a new well S4 -R, which also included additional time. An additional <br />250 calendar days were added to the original 240 -calendar days to incorporate the construction of the <br />new well S4 -R. <br />Analysis: <br />The work was originally intended to be completed within 445 days on February 14, 2019. To date, it <br />appears AWE may be completed more than a year and a half over the originally approved schedule, or <br />approximately 600 days. Following are some of the reasons for delays caused by AWE: <br />• Non responsive <br />• Lack of proper management of the project <br />• Delays transitioning between wells and phases of construction (typically delays starting the next <br />well in sequence and the transition between drilling/acidization/development/testing and <br />wellhead construction) <br />• Equipment issues requiring activities to be performed multiple times and requires multiple trips <br />(geophysical logging and videos). <br />• Delays from not completing well S-7 and S-4 wellheads delayed the start of work at S-2. Slow <br />completion of S-2 wellhead led to delays in starting S-3, etc. <br />70 <br />
