EXTENSION TO INDIAN RIVER COUNTY <br />CARES ACT AGREEMENT <br />THIS EXTENSION TO INDIAN RIVER COUNTY CARES ACT AGREEMENT <br />("Extension") is entered into as of the 5 day of Tari K-xs, , 2021 by and between Indian <br />River County, a political subdivision of the State of Florida, lose address is 1801 27'h Street, <br />Vero Beach, Florida, 32960 ("Recipient"), and Cleveland Clinic Indian River Hospital, a not-for- <br />profit hospital, whose address is 1000 36th Street, Vero Beach, FL 32960 (hereinafter referred to <br />as the "Subrecipient"). <br />RECITALS <br />WHEREAS, Recipient and Subrecipient entered into the Indian River County CARES Act <br />Agreement, which set forth a scope of work for the use of CARES Act funds by the Subrecipient <br />(the "Agreement"); and <br />WHEREAS, the Agreement was to expire on December 30, 2020; and <br />WHEREAS, the Recipient and Subrecipient wish to extend the Agreement until <br />September 30,2021; and <br />NOW THEREFORE, in consideration of the mutual undertakings herein and other good and <br />valuable consideration, the receipt and adequacy of which is hereby acknowledged, the parties <br />agree, as follows: <br />1. Recitals. The above recitals are true and correct and are incorporated herein. <br />2. Extension of the Agreement. The Agreement is extended until September 30, 2021. <br />3.
