Healthy Start Coalition Indian River County <br />NURSE -FAMILY PARTNERSHIP <br />Grant: $10,000 <br />Funds Used: $10,000 <br />Total Families Served: 29 Cost per child: $344.83 <br />Focus Area: Early Childhood Development and Building Parent Capacity <br />Taxonomy Number: PN -8100.6500 Parent/Family Support Groups, PH -6100.1800-900 Teen Expectant/New <br />Parent Assistance. <br />Program Description: Nurse Family Partnership (NFP) is a community health program that helps vulnerable <br />mothers in specific targeted communities achieve a healthy pregnancy. This is attained by helping mothers <br />engage on preventative health practices through early prenatal care, improving their diets, reducing their use <br />of cigarettes, alcohol, and illegal drugs. NFP helps improve child health and development by assisting parents <br />with responsible and competent care and by assisting parents to build a positive future for their children. <br />Mothers enrolled in the program receive free in home visits from a registered nurse from the prenatal period <br />through the baby's second birthday. <br />Collaboration: Cleveland Clinic Indian River Hospital, Partners in Women's Health, Kindergarten Readiness <br />Collaborative, Moonshot Moment. <br />GOALS/OUTCOMES REPORT <br />Outcome # 1: Children and families have improved access to high quality preventative care, measured by the <br />individuals who have increased knowledge of the medical and social service systems. <br />Goal Results: 100% Overall <br />• 100% / 2 out of 2 families will improve their knowledge of medical services available in their <br />community.*Please note that since 2018 to the end of this quarter, 44 out of 47 (total enrolled clients) <br />families achieved this indicator. <br />• 100% / 2 out of 2 families will improve their knowledge of social services available in their <br />community.* Please note that since 2018 to the end of this quarter, 40 out of 47 (total enrolled clients) <br />families achieved this indicator. <br />• 100%/ 2 out of 2 families will demonstrate increased access of services available in their community.* Please <br />note that since 2018 to the end of this quarter, 40 out of 47 (total enrolled clients) families achieved this <br />indicator. <br />Outcome # 2: Families have improved access to high quality services that promote healthy prenatal and <br />postpartum healthy behaviors. <br />Goal Results: 100% Overall <br />• 100% / 2 out of 2 referrals in the targeted community that are eligible for NFP will enroll in services <br />(throughout July 2020 — September 2020).
