Substitute Form W-9 Submission and My Florida Marketplace (MFMP) Registration <br />For the purpose of this Agreement, a Subrecipient is also a Vendor. The State of Florida requires vendors <br />doing business with the State to submit a Substitute Form W-9. The purpose of a Form W-9 is to provide <br />a Federal Taxpayer Identification Number (TIN), official entity name, a business designation (sole <br />proprietorship, corporation, partnership, etc.), and other taxpayer information to the State. Submission of <br />a Form W-9 ensures that the State's vendor records and Form 1099 reporting are accurate. Due to specific <br />State of Florida requirements, the State will not accept the Internal Revenue Service Form W-9. <br />Effective March 5, 2012, State of Florida agencies will not be permitted to place orders for goods and <br />services or make payments to any vendor that does not have a verified Substitute W-9 on file with <br />the Department of Financial Services. Vendors are required to register and submit a Form W-9 on the <br />State's Vendor Website at httns://flvendor.mvfloridacfo.com. <br />Subrecipient must register with My Florida Marketplace utilizing myfloridamarketplace.com website <br />concurrent with the execution of this agreement. Registration must be complete prior to returning this <br />agreement to FDEM for execution. <br />13. Small, Women Owned and Minority Owned Businesses <br />2 CFR 200.321 requires a non -Federal entity take all necessary affirmative steps to assure that minority <br />businesses, women's business enterprises, and labor surplus area firms are used when possible. These <br />affirmative steps must include: <br />(1) Placing qualified small and minority businesses and women's business enterprises on solicitation lists; <br />(2) Assuring that small and minority businesses, and women's business enterprises are solicited whenever <br />they are potential sources; <br />(3) Dividing total requirements, when economically feasible, into smaller tasks or quantities to permit <br />maximum participation by small and minority businesses, and women's business enterprises; <br />(4) Establishing delivery schedules, where the requirement permits, which encourage participation by small <br />and minority businesses, and women's business enterprises; <br />(5) Using the services and assistance, as appropriate, of such organizations as the Small Business <br />Administration and the Minority Business Development Agency of the Department of Commerce; and <br />(6) Requiring the prime contractor, if subcontracts are to be let, to take the affirmative steps listed in <br />paragraphs (1) through (5) of this section. <br />45 <br />
