SPA ITH, C L E R K <br />Alt Forth 525.010-6OeA STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION 52MI l -OA <br />STATE -FUNDED GRANT AGREEMENT PROGRAM MANAGEMENT <br />09120 <br />EXHIBIT A <br />PROJECT DESCRIPTION AND RESPONSIBILITIES <br />FPN: 438074-1-54-01 <br />This exhibit forms an integral part of the Local Agency Program Agreement between the State of Florida, Department of <br />Transportation and <br />INDIAN RIVER COUNTY (the Recipient) <br />PROJECT LOCATION: <br />❑ The project is on the National Highway System. <br />❑ The project is on the State Highway System. <br />PROJECT LENGTH AND MILE POST LIMITS: 1.814 Miles. 6TH AVENUE FROM SR-5/US-1 (MP 0.000) TO 21ST <br />STREET (MP 1.814) <br />PROJECT DESCRIPTION: Milling, resurfacing and pavement markings along with drainage improvements on 6th Avenue <br />from SR-5/US-1 to 21St Street. <br />SPECIAL CONSIDERATIONS BY RECIPIENT: <br />The Recipient is required to provide a copy of the design plans for the Department's review and approval to coordinate <br />permitting with the Department, and notify the Department prior to commencement of any right-of-way activities. <br />The Recipient shall commence the project's activities subsequent to the execution of this Agreement and shall perform in <br />accordance with the following schedule: <br />a) Construction to be completed by December 31, 2023. <br />If this schedule cannot be met, the Recipient will notify the Department in writing with a revised schedule or the project is <br />subject to the withdrawal of funding. <br />SPECIAL CONSIDERATIONS BY DEPARTMENT: <br />The CEI consultant cannot be the Engineer of Record (EOR) for the project. The Department will not execute this <br />Agreement, until the final plans, for the Project, have been approved and until all construction work including a <br />Maintenance map is showing all resurfaced areas are within the existing right of way. In addition, the County will not be <br />able to proceed with construction of the project, until the Department provides the Recipient Notice to Proceed. <br />Page 1 of 1 <br />
