EXHIBIT A
CHILDREN'S SERVICES ADVISORY COMMITTEE
2021-22 GRANT APPLICATION
Agency Name:
Crossover Mission Inc.
Address:
4425 US Highway 1, Vero Beach, FL, 32967, US
Executive Director:
Catherine DeSchouwer
Email:
cdeschouwer@crossovermission.com
Phone:
(772) 257-5400
Program Director:
Catherine DeSchouwer
Email:
cdeschouwer@crossovermission.com
Phone:
(772) 321-0449
Program(s) Name:
Building the Future of Crossover Mission
Focus Area(s):
Quality Academic Remediation and Enrichment
Addressing Risky Youth Behavior
PROGRAM DESCRIPTION:
Crossover helps the kids who need us most, primarily boys, ages 9-18, through a program of
year-round basketball and one-to-one academic mentoring. Included in the Crossover program is
a subset of high-risk students who continue to display disruptive behaviors and experience
ongoing academic failure. In this population, each student is assigned a team and evaluated
weekly. Interventions may include up to 4 additional mentoring days per week, ongoing
communication with the school staff, advocacy in testing/development of Individualized Education
Plan (when indicated), additional support with transportation/medical/legal issues, and in cases
of imminent school drop-out, an online homeschool program administered full-time at the
Crossover Center.
Crossover provides transportation from 14 different schools over the course of each week.
Students who attend a school not on the route, are transported by family members to the Center.
Once at the Center, students receive a hot meal (provided by The Source), participate in the group
Crossover XPress character development program followed by working one-to-one with an
assigned volunteer academic mentor. After tutoring, students are transported to basketball
practice which takes place in gyms throughout the county including Storm Grove and Gifford
Middle School, the Gifford Youth Achievement Center and the Intergenerational Center. Parents
pick up their child at 7pm after basketball training.
The projected number of unduplicated children to be served is ninety (90).
