Ennis, P.E., PMP, Assistant Public Works Director <br />FROM: Kirstin Leiendecker, P.E., Roadway Production Manger <br />■ <br />SUBJECT: FDOTTransportation Regional Incentive Program (TRIP) Supplemental Agreement <br />No. 3 for 66th Avenue Roadway Widening (49th Street to 69th Street) FM No. <br />436379-1-54-01;436379-1-54-02 (IRC -1505) <br />DATE: October 8, 2021 <br />DESCRIPTION AND CONDITIONS <br />On May 7, 2019 the Board of County Commissioners approved Resolution No. 2019-034 and a <br />Transportation Regional Incentive Program (TRIP) Agreement with the Florida Department of <br />Transportation (FDOT) for the widening of 66th Avenue from a two-lane to a four -lane divided highway from <br />49th Street to 691h Street as well as for Construction Engineering Inspection (CEI) services with an estimated <br />cost of the project of $36,700,692.00. The FDOT TRIP grant amount was originally approved for <br />$4,747,018.00 with the County's cost share to be $31,953,674.00 and the agreement was executed by the <br />FDOT on May 22, 2019. Two Supplemental Agreements were subsequently approved by the BCC and the <br />FDOT, revising the estimated cost of the project to $37,226,671.00 and increasingthe grant amount total to <br />$18,613,335.00 for construction and CEI costs with the County's cost share to be $18,613,336.00. <br />Supplemental Agreement No. 3 is to support a time extension to the Terms of the Agreement to December <br />31, 2023. The original Agreement required Construction to be completed by June 30, 2021. Amendment <br />No. 2 extended the contract time to June 30, 2022. The County issued Notice to Proceed to the General <br />Contractor Halley Engineering Contractors, Inc. on June 1, 2021 with a construction contract time to Final <br />Completion of 750 days. A time extension is required to avoid expiration of the agreement prior to project <br />completion.
