If the project will not be completed and all close out paperwork submitted by September <br />30th , a request for a ]-year extension of the completion date of the project should be submitted with <br />the June 2023 quarterly report. <br />SEPTEMBER 1-30,2023 - Work on Closeout paperwork <br />Closeout paperwork consists of : <br />1. Project Completion Certificate, FIND Form No. 90-13 (effective date 7-30-02), which certifies that <br />the project was completed in accordance with the project agreement and the final project plans. <br />2. A final reimbursement request accompanied by all required supporting documentation including <br />bills and canceled payment vouchers for expenditures. <br />3. Photograph(s) showing the installation of the sign required by Rule 66B-2.013, F.A.C. <br />4. Photograph(s) of the completed project clearly showing the program improvements. Opg or of <br />format) <br />5. A Final Project Report (1-2 pages) that shall at minimum include: project name and address, project <br />summary, final cost, and project benefits to the waterway. <br />SEPTEMBER 30, 2023 - End of Grant. All work must be complete closeout paperwork submitted. <br />October 2023- finish processing closeout paperwork, perform project inspection and submit final <br />reimbursement check and coordinates check presentation with sponsor. <br />NOTE: ANY MODIFICATIONS to the PROJECT shall require advance notice and prior written <br />approval of the District. The appropriate timing for modifications to the project cost estimate, <br />Exhibit A, would be after receipt of bids. <br />*NON-COMPLIANCE by the PROJECT SPONSOR with the reporting schedule in Exhibit G <br />may result in revocation of this agreement pursuant to Section 13 of the project agreement. <br />105 <br />
