THIS ORDER FORM ("Order Form") shall outline the products and/or services (collectively, the "Services") to be <br />provided by CommandLink, LLC ("CommandLink") to the customer identified hereii ("Customer"). This Order Form <br />and the delivery, installation and provision of Services shall be subject to CommandLink's Master Service Agreement <br />("MSA") and CommandLink's Service Level Agreement ("SLA"), which can both be found at <br />https://www.command[ink.com/terms-of-service. The MSA and SLA shall be expressly incorporated herein and shall <br />be binding on Customer. <br />If this Order Form is not executed by CommandLink, CommandLink's provisioning of the Services shall indicate its <br />acceptance of the Service Agreement. Customer shall pay CommandLink an Early Termination Fee as set forth in the <br />MSA if Services are terminated prior to the minimum term (" k1in. Term") specified herein. Customer shall be <br />responsible for costs and/or fees associated with additional Services that are performed by, or additional Products are <br />provided by, CommandLink that are not specified herein. <br />This Order Form may be executed in counterparts, each of which shall be deemed an original but all of which shall <br />constitute one and the same instrument. A facsimile, electronic or portable document format ("PDF") transmitted copy <br />of this Order Form executed by one of the parties hereto shall be accepted as a copy of this Order Form originally <br />executed by such party. <br />"MRC" means monthly recurring charge, which Customer can expect to be invoiced for pursuant to the terms of the <br />MSA on a monthly basis.
