0 <br />RESOLUTION NO. 69- 57 <br />The Board of County Commissioners of Indian River County, <br />Florida, resolves: <br />1. Pursuant to the provisions of Chapter 69-8g constituting an <br />amendment to Section 154.06, Florida Statutes, this Board does herewith <br />establish a schedule of reasonable fees which the Indian River County Health <br />Department may collect for services rendered for the following services: <br />Health cards <br />$2.00 each <br />Vital Statistics <br />1.00 a copy <br />Home Nursing <br />8.00 a visit <br />Home Health Aids <br />3.50 an hour <br />2. All funds collected pursuant to this Resolution shall be trans- <br />mitted by the Indian River County Health Department to the State Treasurer <br />for credit to the Indian River County Health Department Trust Fund and all <br />such funds shall be expended solely for the purpose of providing health ser- <br />vices and facilities within Indian River County, Florida. <br />3. The Clerk is hereby directed to transmit a certified copy of <br />this Resolution to the Department of Health and Rehabilitation Services, <br />Division of Health of the State of Florida, the State Treasurer and the Indian <br />River County Health Department. <br />Smith, Heath, Smith & O'Haire, Attorneys At taw, P.
