No: 3124T1 M01202PG <br />Street 3124T1 M012021IRC <br />To: Timothy Rose Contracting, Inc. <br />1360 Old Dixie Highway, Suite 106 <br />Vero Beach, FL 32962 <br />Emergency: ❑ Non -Emergency <br />The contractor's utility material provider and distributer, Star Pipe Products and Core & <br />Main, respectively, have provided the contractor of force majeure event which prohibits them <br />from honoring their original utility material price quotes associated with this project. The <br />justification for this claim was identified to be escalating material costs, supply chain costs <br />and labor shortages due to Covid-19 Pandemic. As such, the utility material and distributer <br />have provided revised material costs necessary to support the utility improvements <br />assocaited with this project. <br />2. During the construction of 58th Avenue north of 57th Street, an existing air release valve <br />(ARV) was located that had previously not been identified. The existing ARV was located <br />beneath approximatley 2.0 -feet of soil. The County has requested that the ARV be relocated <br />to be within the proposed raised median located along 58th Avenue. <br />3. During the constructon of the proposed 20" water main improvements located north of the <br />IRFWCD Main Relief Canal, County Utility Staff requested that the contractor modifiy the <br />proposed connection point further to the north approximately 25 -ft. The reasoning for the <br />modification request was to remove several existing abandoned utility appurtenances <br />located on the existing main. The abandoned appurtenances would have been located <br />beneath the newly constructed pavement and were viewed as a source of future mainteance <br />issues. <br />4. During the construction of the 18" culvert located between S-136 and S-137, it was identified <br />that the existing 8" PVC force main was constructed approximately 4.5 -feet below existing <br />grade. As such, the existing utility created a conflict with the proposed drainage <br />improvements. To resolve the conflict, it was determined that extending the proposed 10" <br />HDD force main bore was the preferred solution. <br />The following summarizes the additional project costs associated with each item identified <br />above: <br />1. Utility Material Escalation Cost = $ 135,617.97 <br />2. Sta. 175+00 ARV Relocation = $ 8,400.00 <br />3. 20" Water Main Revision = $ 28,931.00 <br />4. 10" FM HDD Extension= 100 if x $1G6.04/If = $10,604.00 <br />Contract unit pricing has been used when identified within the contract bids. Where contract <br />pricing was not available the contractor has provided detailed cost proposals. <br />92 <br />
