ab <br />40 <br />RE S 0 L U T I ON 70"-58 <br />BE IT RESOLVED by the Board of County Commissioners I <br />of Indian River County, Florida, that the said Board does hereby;, <br />elect, on behalf of _Ralph Harris, Clerk Circuit Court, Indian River County <br />I' <br />a fee basis officer of Indian River County, to exercise the <br />authority provided by Chapter 65-1714, F.S. as that the above— <br />mentioned fee basis officer may enter into an agreement with i <br />Blue Cross — Blue Shield of Florida under"Policy 11 -PK with <br />i <br />Extended Benefits" carrying monthly premiums of $ 9,95 for a <br />single rate and $ 27.06 for a family rate; with thn said fee i <br />basis officer contributing $ 9.95 for each single rate I <br />employee contract, and $15.00 for each family rate employee <br />contract, as operating expense'of hia offioei and the balance i <br />of the monthly premium, if any, to be paid by the individual <br />employee by means of payroll deduction. <br />r <br />
