Ennis, P.E., PMP, Assistant Public Works Director <br />FROM: Mark Winslow, P.E., Project Engineer <br />SUBJECT: Work Order No. 4—Andersen Andre Consulting Engineers, Inc. <br />IRC -1639 - 69th Street From 66th Ave. to SRS/US1, Resurfacing <br />Construction Materials Testing Services <br />DATE: June 10, 2022 <br />DESCRIPTION AND CONDITIONS <br />On December 7, 2021, the Board of County Commissioners renewed and amended the Continuing Contract <br />Agreement for Continuing Geotechnical Engineering, with Andersen Andre Consulting Engineers, Inc. for <br />various services including, but not limited to, field and laboratory construction materials testing in Indian <br />River County. <br />The purpose of Work Order No. 4 is to provide construction materials testing services for the resurfacing of <br />69th Street from 66th Avenue to SR5/US-1. Andersen Andre Consulting Engineers, Inc. submitted the <br />proposal outlined in the attached Work Order No. 4 and Scope of Services (Exhibit A) for a not -to -exceed <br />amount of $46,510. <br />FUNDING <br />Funding is budgeted in Account Number 10921441-066510-19003, Secondary Roads/69th Street/66th Ave to <br />US1 for $46,510. <br />RECOMMENDATION <br />Staff recommends approval of Work Order No. 4 to Andersen Andre Consulting Engineers, Inc. authorizing <br />the professional services as outlined in the Scope of Services (Exhibit A) and requests the Board to authorize <br />the Chairman to execute Work Order No. 4 on their behalf for a not -to -exceed amount of $46,510. <br />ATTACHMENTS <br />Work Order No. 4 <br />Scope of Services (Exhibit A) <br />AGENDA ITEM FOR JUNE 21, 2022 <br />61 <br />C:\Gra n icus\Legistar5\L5\Temp\74089d37-3a2d-4971-9655-1b980543dffa.doc <br />
