Alt Form 525 010-60eA s LATE_OF FLORIDA DEPARTMENT OF TRANSP04l IA I ION 525-011-0A <br /> STATE-FUNDED GRANT AGREEMENT PROGRAM MANAGEMENT <br /> 05!21 <br /> EXHIBIT A <br /> PROJECT DESCRIPTION AND RESPONSIBILITIES <br /> FPN: 431759-2-54-01 <br /> This exhibit forms an integral part of the Agreement between the State of Florida, Department of Transportation and <br /> Indian River County(the Recipient) <br /> PROJECT LOCATION: <br /> ❑The project is on the National Highway System. <br /> ®The project is on the State Highway System. <br /> PROJECT LENGTH AND MILE POST LIMITS: .090 miles <br /> 88060000 28.088 to 28.178 <br /> PROJECT DESCRIPTION: Construction for the Project will include, but not be limited to the following: <br /> • Roadway reconstruction @ SR-60&43rd Ave <br /> •Drainage Improvements <br /> •Traffic Signal Improvements <br /> •Signing and Pavement Markings <br /> • Installation of new mast arms <br /> • Lighting <br /> • Landscaping <br /> •ADA upgrades to all curb ramps,sidewalks,and curb and gutters within the Project's limits <br /> • Polyurethane injection to fill the voids and stabilize the surrounding soils beneath the roadway surface. <br /> The COUNTY shall perform the following services at their sole cost and expense: <br /> • Continuation of Professional Engineering Services for the reconstruction of the intersection al SR-60 and 43rd Ave.(Post <br /> Design) <br /> • Utility work and/or relocation <br /> SPECIAL CONSIDERATIONS BY RECIPIENT: <br /> The Recipient is required to provide a copy of the design plans for the Department's review and approval to coordinate <br /> permitting with the Department, and notify the Department prior to commencement of any right-of-way activities. <br /> The Recipient shall commence the project's activities subsequent to the execution of this Agreement and shall perform in <br /> accordance with the following schedule: <br /> a)Construction to be completed by June 30, 2023 <br /> Page 1 of 2 <br />
