S TAU OF F(ARIDA D(PAR EMI N 1 01 IRANSPOR(A 1 ON 575 01060 <br /> STATE-FUNDED GRANT PROGRAM MANAGE MENT <br /> 17;18 <br /> SUPPLEMENTAL AGREEMENT Page 7 of 3 <br /> Paragraph 3-Term A is amended to read as follows: <br /> A. Except as otherwise set forth herein, this Agreement shall continue in effect and be binding to all parties until the <br /> Project is completed as evidenced by the written acceptance of the DEPARTMENT and the COUNTY or June 30, 2023, <br /> whichever comes first. <br /> Reason for this Supplemental and supporting engineering and/or cost analysis: <br /> 1)The Exhibits in the Original Agreement and subsequent Amendments are updated/amended pursuant to this SA#3 to <br /> be in compliance with the most updated and current financial and audit terms and conditions. <br /> 2)The additional State compensation under this SA#3 is the result of unforeseen, pre-existing conditions that affects the <br /> Project construction. During construction of the 43rd Ave&SR60 Intersection Improvements a drainage trunk line <br /> located under eastbound SR60 was found to have leaks. The County's contractor will perform Polyurethane injection to <br /> fill the voids and stabilize the surrounding soils beneath the roadway surface. <br /> 3)The reason for the time extension is to provide sufficient time for the contractor to complete the work for the unforseen <br /> conditions; and to allow the contractor sufficient time to complete the Project before the State Funded Grant Agreement <br /> expires. <br /> THIS SPACE IS INTENTIONALLY LEFT BLANK. <br />
