STATE OF FLORIDA DEPARTMENT OF TRANSPORTATION <br />STATE -FUNDED GRANT <br />SUPPLEMENTAL AGREEMENT <br />SUPPLEMENTAL NO. <br />1 <br />CONTRACT NO. <br />G1U61 <br />FPN <br />438073-1-54-01 <br />Recipient: Indian River County <br />This Supplemental Agreement ("Supplemental'), dated <br />525.010-60 <br />PROGRAM MANAGEMENT <br />12/- 8 <br />Page 1 of 2 <br />arises from the desire to supplement the <br />State -Funded Grant Agreement ("Agreement") entered into and executed on February 11, 2021 as identified above. All <br />provisions in the Agreement and supplements, if any, remain in effect except as expressly modified by this Supplemental. <br />The parties agree that the Agreement is to be amended and supplemented as follows: <br />This Supplemental Agreement extends the Term of the Agreement from December 31, 2022 to December 31, 2023. <br />The first sentence of Paragraph 3 is amended to read as follows: <br />This agreement shall commence upon full execution by both Parties and the Recipient shall complete the Project on or <br />before December 31, 2023 <br />The deliverables stated in Exhibit A in the original agreement dated February 11, 2021 are hereby deleted and replaced <br />with Exhibit A deliverables of this SA #1. <br />Reason for this Supplemental and supporting engineering and/or cost analysis: <br />Due to a Project redesign and material procurement delays, the Deliverables need to be updated and a time extension is <br />is needed to provide sufficient time for the contractor to complete the Project before the State Funded Grant Agreement <br />expires. <br />80 <br />
