650 GARDEN COb1AiERCE PARKWAY, SUITE 100 <br />TOHN (SON idh LA V Y`11 <br />WINTER GARDEN, 34787 <br />J v U PHONE 407.77 70.2180 <br />CON STRUCTION FAx 407.770.2181 <br />09/01/2022 <br />PROJECT: Indian River County: Archie Smith Fish House Pavilion <br />SUBJ: Scope of Work <br />Summary Scope: <br />This project consists of new decking and a pavilion at the Archie Smith Fish House located at 1740 North Indian River Drive Sebastian, FL <br />32958. <br />Detailed Scone of Work: <br />Provide labor, material, and equipment in accordance with the site walk performed on 7/28/2022 and the reference drawings by Atelier <br />Architecture, Engineering, Construction Incorporated dated 7/28/2014. <br />Base Bid: <br />Design and engineering for new deck, foundation, and pavilion <br />- New deck and pavilion to go in footprint of where the existing Ice House was located (approximately 21' x 31') <br />o Light fixtures in the pavilion and light poles on the walkway are excluded <br />o Painting is not included <br />- New wood railing to match the existing railing <br />Alternate: <br />- Demo of the existing Fish House <br />Reconfigure and provide new chain link fencing to match existing <br />- Salvage signage and shiplap for repurpose <br />o Salvage materials to be turned over to the county after removal <br />o Contractor not responsible for storage or transporting salvage material <br />- Asbestos testing of the existing of Fish House. Any abatement to be addressed in a supplemental proposal pending initial test <br />results <br />Clarifications: <br />Work to be performed during normal/regular business hours as defined: Monday through Friday, 7:00am to 3:30pm with holidays <br />excluded <br />Price proposal subject to change upon review of the engineered design drawings and details <br />Owner is responsible for all power, water, HVAC, and wastewater safe -off and disconnection for the existing Fish House <br />Building permit fee allowance included <br />Any special or environmental permits/notices such as FDEP, Corps of Engineers, etc. to be done by others <br />SWPPP management plan and water quality monitoring not included <br />Dumpster to be provided by GC <br />Davis -bacon wages / Certified payroll are not included <br />Liability insurance included <br />Builder's risk insurance included <br />Long Shoreman's Insurance included <br />Bond included <br />As-Builts are included <br />No Liquidated Damages <br />Any unforeseen conditions are excluded <br />- Johnson-Laux Construction is not responsible for delays to the schedule caused by others <br />www.johnson-laux.com <br />C(3C1519701 <br />
