+0 <br />40 <br />11.8. All supporting information requested in the [yid Form must be furnished. Do not <br />leave any questions or requests unanswered. <br />12. 5,uhmission of Bids <br />12.1. Bids shall be submitted at the time and Palace indicated in the Invitation to Sid <br />and shall be included in an opaque seated envelope, marked with the Project <br />title and name and address of the Bidder and accompanied by a completed <br />Sworn Statement under the Florida Trench Safety Act, Statement on Public <br />Entity Crimes, the Bid Security and other required documents which are <br />included in the Contract Document. If the Bid is sent through the mail or other <br />delivery system, the sealed envelope shall be enclosed in a separate envelope <br />with the notation "Bid Enclosed" on the fuco thereof. <br />12.2. The successful Bidder will be responsible for compliance with all applicable <br />safety-related Federal and State statutes and regulations, including, but not <br />limited to, the OSHA excavation safety standards, 20 C.F.R., Part 1920, <br />Subpart P -Excavations, Trenching and Shoring, which are adopted by <br />reference of the newly -enacted Florida "Trench Safety Act", Section 553.60 et. <br />Seq., Florida Statutes The Bidder who will perform trench excavation work on <br />a construction project with trench excavation(s) over 5 feet in depth must <br />allocate and include in its bid the cost of compliance with trench safety <br />standards and any special shoring requirement. The Bidder must indicate in <br />the sworn statement the method(s) he intends to use to achieve compliance <br />with each item (i.e., trench box, sheet piling, cribbing, wood shoring, or other <br />method selected by Bidder). <br />13. Modification and Withdrawal of Bids <br />13.1. Bids maybe modified or withdrawn by an appropriate document duly executed <br />(in the manner that a Bid must be executed) and delivered to the place where <br />Bids are to be submitted at any time prior to the opening of Bids. <br />13.2. If, within twenty-four hours after Bids are opened, any Bidder files a duly signed <br />written notice with Owner and promptly thereafter demonstrates to the <br />reasonable satisfaction of Owner that there was a material and substantial <br />irnisiake In the pi v. a(ation of his Bid, Thai Bidder may withdraw nis bid and the <br />Bid Security will be returned. Thereafter, that Bidder will be disqualified from <br />further bidding on the Work. <br />•. .... :+ <br />Bids will be opened publicly.
