40 <br />40 <br />ATTACHMENT B <br />SCOPE OF WORK <br />CONSTRUCTION AND DEMOLITION (C&D) DEBRIS PERMITTING SERVICES <br />INTRODUCTM <br />Indian River County currently operates a Construction and Demolition (C&D) Debris Landfill <br />facility under a Florida Department of Environmental Protection (FDEP) general operating <br />permit (Permit No. S031-236459). CDM is currently working with FDEP to renew the C&D <br />Debris Operating permit for the landfill. Camp Dresser & Mckee Inc. submitted a groundwater <br />monitoring plan, which includes installation of additional monitoring wells, as part of the permit <br />application package. The previous proposal was budgeted for installation of upgradient and <br />downgradient groundwater monitoring wells based on the installation of four (4) shallow <br />groundwater monitoring wells. During the permitting process the FDEP is requiring an <br />additional 4 intermediate wells and 1 deep monitoring well. This proposal covers the cost of <br />installation of 4 additional intermediate depth monitor wells and 1 deep monitoring well. <br />The new monitoring wells will be tied into existing horizontal and vertical control using Indian <br />River County's contract professional land surveyor. The survey of the monitoring wells is <br />outside the scope of this contract. <br />Task 100 - INSTALLATION OF MONITORING WELLS <br />CDM will subcontract the installation of the monitoring wells to a qualified geotechnical <br />engineering firm. A total of four intermediate and one deep groundwater quality monitoring <br />wells are budgeted in this proposal as required by FDEP. The monitor wells will be installed in <br />accordance with the specific conditions of the current FDEP operating permit for the Landfill. A <br />figure showing the approved number and location of monitoring wells is attached. The specific <br />conditions from are restated below: <br />15. New or replacement monitoring well design or placement must be approved by the <br />Department. Proposed well construction details based on site specific borings must be <br />submitted with all supporting data (grain size, analyses, in-situ hydraulic conductivity testing, <br />death to water, etc.) for Department annroval prior to well installation- I Ne! of hollow Story, <br />auger equipment is recommended. Other drilling methods must be approved by the <br />Department prior to well installation. <br />18. Well completion reports for new monitor well(s) must be submitted to the Department <br />on the attached Ground Water Monitoring Well Completion Report Form within thirty (30) <br />days after installation. Note that the latitude and longitude in degrees, minutes and seconds <br />of each well must be provided on the form. In addition, as -built well construction diagrams <br />and soil boring logs that cover the entire depth of the monitoring wells must be submitted to <br />the Department. <br />1,0169.wpd n.1 <br />
