C-] <br />40 <br />Permittee: INDIAN RIVER COUNTY <br />Permit No: 19904088(IP-IS) <br />2. The applicant agrees to submit a mitigation monitoring plan, <br />within 30 days of issuance of this permit, for the wetlands <br />enhanced/restored by the repair of the dike. The plan should <br />include baseline information such as existing water elevations and <br />plant communities. The plan will require written approval by the <br />corps and shall be initiated concurrently with the construction <br />authorized by this permit. <br />3. The permittee agrees that all monitoring reports generated by <br />the mitigation plan will be mailed to the Merritt Island <br />Regulatory Field office at 2460 North Courtenay Parkway, Suite <br />101, Merritt Island, Florida 32953. Each monitoring report must <br />include the following items: <br />(1) the Department of the Army Permit number, <br />(2) the sequence number of the report being submitted, <br />(3) the date the next report is expected to be submitted, and <br />(4) a brief summary of the status of the mitigation including <br />any problems encountered and the remedial actions taken. <br />4. As preventative measures for lead contamination at the project <br />site, the permittee agrees to abide by the SJRWMD permit <br />#4 -067. -0174G -ERP special conditions numbers 26 through 29 <br />(Attachment #2). <br />5. The applicant has agreed to develop an Eastern indigo snake <br />protection/education plan for the review and approval of the Fish <br />and Wildlife Service at least thirty (30) days prior to the <br />commencement of construction or clearing activities. The plan <br />should be submitted to the attention of ties Shockley, FWS, P.O.. <br />Box 2676, Vero Beach, Florida 32961-2676.
