CDBG-E-8 <br />HISTORIC PRESERVATION QUESTIONNAIRE <br />A. Are there any structures, public. improvements, or planned open spaces to be affected by the proposed <br />activities that are more than 50 years old? Yes No -,x <br />The question should be answered 'yes' If: <br />1) The project or any activity will result in any direct physical change to structures that are more than <br />50 years old, including demolition (partial or complete), rehabilitation, restoration, remodeling, <br />renovation, expansion, or relocation; <br />2) The project will result in direct physical change to public improvements that are more than 50 years <br />old, such as stone curbs or brick streets, <br />3) The project Will result in direct physical change to planned open spaces (.e., park/ plaza) that are <br />more than 50 years old; or <br />4) Project activities wilt occur within 100 feet of a structure, public Improvement, or planned open <br />space that is more than 50 years old. <br />NOTE: Projects or components thereof that affect properties that are listed or eligible for listing in the National <br />Register of Historic Ptaces shall be designed in accordance with the recommended approaches in the Secretary of <br />the Interior's Standards or Rehabilitation Guidelines for Rehabilitating Historic Buildings (U.S, Department <br />of the Interior, National Park Service, 1983). Copies of this publication and technical assistance regarding the <br />application of these Standards and Guidelines to specific projects may be obtained from the State Historic <br />Preservation Office (SHPO) at the address on page 1 of the Application Deadline and Instructions. Local <br />governments with proposed activities affecting structures, public improvements, and planned open spaces that are <br />more than 50 years old are encouraged to contact the SHPO as soon as possible in the project planning process, <br />a.
