40 <br />C> <br />• <br />THREE PALMS OF SEBASTIAN <br />70 BED ALF <br />PROJECTED STAFFING <br />FIRST SEVENTEEN MONTHS OF OPERATION <br />30 Beds Dementia Care <br />44 Beds Regular Extended Congregate Care <br />Assumptions <br />There are no residents for the first month after the facility construction is complete. At the <br />end of that time, five residents would begin occupancy, and an additional four to five would enter <br />each month at the beginning of the month, At the end of the first six months, there would be a total <br />of 25 residents, of which 14 would be in the dementia care unit. After six months, new occupancy <br />would decrease to four per month, due to the initiation of turnover in original occupancy, with one <br />of the four being deme=ntia care. full occupancy of 63 (assumes 9U°/ooccupancy) will be achieved <br />by the beginning of the seventeenth month, with 27 in dementia care and 36 in regular care, <br />All of the residents would be receiving a moderate to high level of personal assistance, <br />involving at least two ADLs (such as supervision of self- administered medication, and bathing). The <br />staff perform all necessary tasks during the initial period of occupancy, regardless of job description, <br />with the exception of administrative or other duties which must be performed by the administrator <br />or nurse. All staff members vAll be certified in first aid and complete a course on UUV and AIDS in <br />the workplace within one month of employment. Special training will be provided in the area of <br />dementia care. Criminal background, abuse registry, driving record, and workers compensation <br />claims checks, as well as drug use testing will be performed on employment applicants before <br />employment, in addition to ,general employment reference checks. RN will be utilized on a part time <br />basis as a supervisor for the LPNs. <br />First Month, 0 residents <br />There must be six initial employees of the facility. One is the administrator, who will be marketing <br />and finalizing other arrangements such as supplier contracts and social services. An RN will be on <br />the payroll as part time, for licensing purposes only at this stage, but marketing tasks will be assigned <br />until actual nursing services are needed. Another is the administrative/financial assistant. A CNA <br />will be hired and trained during the month (with emphasis on dementia care), during which time she <br />will also do cleaning. A half time lawn and maintenance person will also be hired. <br />Second Month, 5 residents <br />A full time social director will be hired and trained (emphasis on dementia care) this month. The <br />social director function will be important for marketing/sales purposes as well as service delivery, <br />and will relieve the administrator of the burden of weekend work All social activities (including <br />shopping and other excursions) will be planned and supervised by this employee, who will also <br />perform other tasks as necessary to assist the administrator and provide personal care to residents, <br />
