AND <br />INDIAN PIVrR COUNTY, FLORIDA, <br />This is an amendment to the existing Engincering/Land Surveying Services Agreement <br />(AGREEMENT) dated November 1, 1994, between Kinilcy-flom and Associates, Inc. <br />(ENGINEER) arid Indian River County (COUNTY). This aniendincrit addresses changes in <br />"Section I - Project Limits and Description" and "Section V - Compensation", of the <br />AGREEMENT. <br />Amendment D <br />tRqpmri tion <br />This Amendment includes the following: <br />"Section I - Project Limits and I)c,&cription" is being modified to incorporate structural <br />design review services for proposed modifications of design plans by the contractor <br />during King's Highway (58" Avenue) construction between 12' Street and 26' Street. <br />We understand that these modifications are being requested and initiated by the <br />contractor, and that the County will be rcimbktrsictl by the contractor tor any professional <br />review ansing from these modifications. <br />We will review design joidilications submitted by the contractor to include: <br />• Concrete shed piles <br />• Modiricationswidilisofticckslash tulits <br />Closure pours <br />• End bent wing wall modifications <br />We will also review the shop drawing submittals associated with these <br />modifications and coordinate any comments or revisions with the contractor to <br />assist in expediting his modification. <br />"Section V - Compensation" is being revised in response to the Scope <br />modifications. 'fire charges associated with the change in project scope arc as <br />follows: <br />1) I)esigo review and shop drawing review 510,575,00 <br />Expenses will be charged at a lump sum of 11 % of labor cost for a total of $740. <br />The section of the original ACMEEMNT entitled "Section V - Compensation" <br />shall be revised to include compensation due the ENGINEER in the amount of <br />$11,315. <br />Page I of 2 <br />
