• <br />k. <br />M <br />11.4. Bids by partnerships must be executed in the partnership name and signed <br />by a partner, whose title must appear under the signature and the official <br />address of the partnership must be shown below the signature. <br />11.5. All names must be typed or printed below the signature. <br />11.6, The Bid shall contain an acknowledgment of receipt of all Addenda (the <br />numbers of which shall be filled in on the Bid Form), <br />11.7. The address to which communications regarding the Bid are to be directed <br />must be shown. <br />11.€3. All supporting information requested in the Bid Form must be furnished. Do <br />not leave any questions or requests unanswered. <br />a- <br />12.1. Bids shall be submitted at time time and place indicated in the Invitation to Bid <br />and shall be included in an opaque sealed envelope, marked with the <br />Project title and narne and address of the 'Bidder and accompanied by a <br />completed Sworn Statement under the Ftorida Trench Safnty Act, Statement <br />on Public Entity Crimes, the Bid Security and other required documents <br />which are included in the Contract Document. If the Bid is sent through the <br />mail or other delivery system, the sealed envelope shall 'be enclosed in a <br />separate envelope with the notation "Bid Enclosed, on the face thereof. <br />12.2. The successful Bidder will be responsible for compliance with all applicable <br />safety-related Federal and State statutes and regulations, including, but not <br />limited to, the OSHA excavation safety standards, 20 C.F.ft., Part 1926, <br />Subpart P -Excavations, Trenching and Shoring, which are adopted by <br />reference of the newly -enacted Florida "Trench Safety Act", Section 553.60 <br />et. Seq., Florida Statutes, The bidder who will perform trench excavation <br />work on a construction project with trench excavation(s) over a feet in depth <br />must allocate and include in its bid the cost of compliance with trench safety <br />standards and any special shoring requirement.
