EXHIBIT A <br />the mental, physical, and emotional needs of pregnant mothers, partners, and families through direct <br />education and outreach. <br />Healthy Families (HF) is modeled after the national HF America; an initiative that is based on critical <br />program elements that have been defined by over 20 years of research and represents "best practices" <br />in home visitation. The national Healthy Families initiative has demonstrated success and received <br />national recognition. <br />The Nurse Family Partnership (NFP) program provides services for first time, low income mothers, <br />fathers and their children. This program provides support to the most vulnerable families in IRC to <br />prevent Fetal, Infant and Maternal mortality. The programs evidence -based research shows NFP <br />support can change the trajectory of a family and future family members. <br />The Fatherhood (T.E.A.M. Dad) program is designed to provide education, group, mentoring and one on <br />one home visitation support to fathers and partners utilizing the evidence -based curriculum 24/7 Dad. <br />7 <br />
