The <br />program targets children ages 5-17 from high poverty communities. The program is currently providing <br />academic support at two neighborhood hubs located in high poverty communities: Oslo (South County) <br />and West Wabasso Outreach Center (North County). The program serves student two days a week at both <br />locations. At the South site we are there on Tuesdays and Thursdays. At the North county site services are <br />provided on Mondays and Wednesdays. The hours at both locations are from 3:30- 6:OOpm. The program <br />provides students access to an online learning system designed to help students reach and maintain <br />proficiency. Students are assessed initially at the beginning of each year or when entering into the program. <br />Afterwards, each student received an individual action plan which is specific to their needs. Students are <br />provided a continuous assessment which modifies the action plan to remove skills students have mastered. <br />During this time students are also provided academic support in small groups. They receive help with <br />homework, foundational skills, project support, and sight word practice. <br />Student progress is monitored using data collected from IXL and focus. Various levels of student progress <br />are monitored to determine if students are meeting their intended outcomes. Student progression is <br />monitored on a weekly basis by teachers. On a monthly basis gains in the continuous diagnostic are <br />noted. Quarterly students complete a Snapshot Assessment to determine the amount of growth since <br />beginning the program. The data collected is compared to results of FAST and (Ready assessments. <br />2 <br />
