40 <br />0 <br />ATTACHMENT I <br />PROGRAM SPECIFIC REPORTING REQUIREMENTS AND PROGRAMS REQUIRING <br />COMPLIANCE WITH THE PROVISIONS OF SPECIFIC MANUALS <br />Some health services must comply with specific program and reporting <br />requirements in addition to the CIS/HMC minimum data set and the SAMAS 2.2 <br />requirements because of federal or state law, regulation or rule. If a <br />county health department is funded to provide one of these services, it <br />must comply with the special reporting requirements for that service. The <br />services and the reporting requirements are listed below: <br />Service <br />1. Sexually Transmitted Disease <br />Program <br />Dental Health <br />3. Special Supplemental Food <br />Program for Women, Infants <br />and Children. <br />Improved Pregnancy Outcome <br />Family Planning <br />Requirement <br />Requirements as specified <br />in HRSM 150-22*. Requirements <br />as specified in Policy 87-7-5 <br />regarding State Health Office <br />STD Program review and approval <br />of personnel/budget actions. <br />Monthly reporting on HRSH <br />Form 1008*. <br />Service documentation and <br />monthly financial reports <br />as specified in HRSM 150-24* <br />and all federal, state and county <br />requirements detailed in the <br />program manuals and published <br />procedures. <br />Requirements as specified in <br />HRSM 150-13A*. Quarterly reports <br />of services and outcome on HRSH <br />Form 3096*. Program Quarterly <br />Progress Report, Quarterly Summary <br />Report, Presumptive Eligibility/ <br />Medicaid Determination Log by all <br />providers authorized to determine <br />presumptive eligibility. <br />Periodic financial and programmatic <br />reports as specified in HRSM <br />150.27*. <br />
