Zito, Assistant County Administrator <br />From: Bela Nagy, Golf Professional <br />Subject: Additional Service No. 4, for Additional Civil Engineering Services — St John's <br />River Water Management District Permit Modification for Sandridge Golf <br />Club - New Clubhouse <br />DESCRIPTION AND CONDITIONS: <br />On October 1, 2021, Indian River County and Sandridge Golf Club entered into an agreement with <br />Sun Patrick Architecture, Inc., DBA Sparcdesign for professional services for the Sandridge Golf <br />Club Clubhouse through RFQ 2021020. Sparcdesign has submitted an additional service request <br />#4 for additional scope of work for the Engineer to certify the existing master system is constructed <br />and functioning as designed and permitted. <br />Sparcdesign has proposed to perform this work as an additional service utilizing the terms outlined <br />in the Agreement for Professional Services for Sandridge Clubhouse Architectural Design Services <br />— RFQ2021020-Master Plan Study Phase dated October 1, 2021. The cost for the additional civil <br />engineering services is a fixed fee of $4,620.
