ATTACHMENT B <br />AMENDMENT NO.4 <br />to <br />WORK AUTHORIZATION <br />between <br />INDIAN RIVER COUNTY <br />and <br />CAMP DRESSER & McKEE INC. <br />Providing <br />PROFESSIONAL SERVICES RELATED TO <br />CENTRAL REGION WASTEWATER TREATMENT PLANT <br />CDM PRO)ECT NO. 6706-13899-070 <br />The Amendment No. 4 dated to the Work Authorization dated November 14, <br />1995 between Indian River County and Camp Dresser & McKee Inc., in consideration of the <br />mutual terms, conditions, promises, covenants, and payments set forth, agree to the following <br />changes: <br />1. Attachment B, Central Region Wastewater Treatment Plant, Task 600 - General Services <br />During Construction is hereby amended as follows: <br />Add the following words to the end of the task: <br />"J. Engineer will provide general engineering services due t ) extended construction <br />duration (two (2) additional months) of the Central Region WWTP to 2.0 Mgd." <br />2. Attachment B, Central Region Wastewater Treatment Plant, Task 700 - Resident Project <br />Representative is hereby amended as follows: <br />Add the following words to the end of the task: <br />"E. Engineer will provide additional resident project representative inspection <br />services for the remainder of construction (additional ten (10) hours per week for <br />three (3) months and thirty (30) hours per week for two (2) months) of the Central <br />Region WWTP to 2.0 Mgd." <br />3. Article IV, Compensation for Services, is hereby amended as follows: <br />Add the following words to the end of the Compensation for Services: <br />"Total Contract (includes Amendment Nos.1 and 3) $725,100 <br />General Services During Construction Extension (Amendment No. 4) 20,530 <br />Resident Project Representative Extension (Amendment No. 4) 18,560 <br />Total Adjusted Contract " <br />,b2e78\:2/29/99 B-1 <br />
