i <br />40 <br />• <br />(a) The dangers of drug abuse in die workphece; <br />(b) The grantees policy of maintaining a drug-free workplace; <br />(c) Any avaltable drug, counseling, rehabilitation, and employ- <br />ee assistance programs: and <br />(d) The penalties that may be imposed upon employees for <br />drug -abuse violations occurring in the workplace; <br />(iii) Making it a requirement that each employee lobe <br />engaged In the performance of the grant fie given, a copy of <br />the state ant required by paragraph (t); <br />(10 Notifying the employee in die statement required by <br />paragraph (i) chat, as a condILon of exnp)oyment under die <br />grant, the employee will — <br />(a) Abide by the terns of doe statement; and <br />(b) Notify the employer in writing of his other conviction for <br />a violation of a criminal drug statute oecuurs mg in the work- <br />place no later than five raleadar days after such conviction. <br />(v) Notifying the ag®tcy, in writing, within 16 calendar days <br />after receiving notice under subparagraph (iv)(b) from an <br />employee or otherwise receiving actual notice of such convic- <br />tion.
