C-1 <br />ATTACHMENT V <br />DESCRIPTION OF USE OF CHD TRUST FUND BALANCES <br />FOR SPECIAL CAPITAL PROJECTS, IF APPLICABLE <br />(From Attachment II, Part I) <br />Dental Expansion Project, $100,072, to supplement the funding <br />allocated for expansion of existing dental services. The health <br />department funding will be dedicated to furnishing and supplying <br />the new area. The project will be completed no later than August <br />15, 2001. <br />Replacement of outdated and failing phone system, $72,000. The <br />project will be completed no later than October 31, 2000. <br />Addition of air handler for first floor, $40,000, to correct <br />indoor air problems in the clinic area. The county is also <br />contributing to this project. The project will be completed no <br />later than December 31, 2000. <br />Complete re -carpeting health department, i.e., Environmental <br />Health, front stairway, and Auditorium, $15,000. The project will <br />be completed no later than May 31, 2001. <br />DESCRIPTION OF SPECIAL CONTRACTS <br />(From Attachment II, Part III) <br />Please list separately <br />Special contracts are contracts for services for which there are no comparable <br />srjrvices in the county health department core programs; no service codes in <br />Departmental coding manuals; projects that are locally designed and have no <br />standard statewide set of services and therefore cannot be accounted for <br />within existing county health department programs. These contracts are coded <br />to FLAIR bevel 5:599 and include some contracts formerly handled at the <br />district offices such as epilepsy, colposcopy, Project WARM, community <br />planning and special family planning and teen mother projects. <br />:1 <br />
