AGREEMENT <br />THIS AGREEMENT is made this 5th day of December, 2040, by and between the <br />BOARD OF COUNTY COMMISSIONERS OF INDIAN RIVER R COUNTY, FLORIDA, 184025th <br />Street, Vero Beach, Florida 32960 (the "County") and ROBIN A. LAPP, LMHC., 545 461i Court, <br />Vero Beach, FL 32968 (the "Contractor'). <br />L SCOPE OF SERVICES <br />A. The Contractor shall provide a comprehensive range of substance abuse, mental <br />health, and educational activities to Indian River County male and female youth during the period <br />of their probation without regard to race, color, creed, national origin, sex, or economic status <br />pursuant to the Anti -Drug Abuse Act of 1988 and the application submitted by the County to the <br />StateofFbrida, Florida Department of Law En forcement, office ofCriminal Justice Grants, ("grant <br />agreement"). The Contractor will coordinate with the Court and Office of Probation and Parole and <br />the Substance Abuse Council of Indian River County to identify adolescents for services. The <br />contractor shall maintain, at her sole expense, during; the term of this agreement, a State of Florida <br />license to practice as it licensed mental licalth counselor. <br />B. If, after the substance abuse problem assessment is conducted, the Contractor <br />determines that the individual meets program eligibility requirements, the Contractor will make <br />available to the individual the following range of services: <br />1. Full psycho -social evaluation <br />2. Individual substance abuse counseling and education <br />3. Group substance abuse counseling; and education <br />4. Mental/emotional health evaluation and counseling. <br />5. Life management skills assessment <br />6. Weekly aftercare groups for a period of no less than 6 months <br />7.. Case Management <br />C. The Contractor shall pcifonn and maintain records consistent With the grant <br />agreement. Said records shall be made available to the County upon request. <br />D.
