If billing for overtime and backfill, has a spreadsheet been provided that lists attendee names, <br />department, # of hours spent at exercise, hourly rate and total paid to each attendee? <br />Have documentation from entity's financial system been provided to prove attendees were <br />paid? <br />For backfill, has a clear delineation/cross reference been provided showing who was <br />backfilling who? <br />❑ 4. Have the names on the sign -in sheets been cross-referenced with the names of the individuals for <br />whom exercise reimbursement costs are being sought? <br />❑ 5. Have any expenditures occurred on supplies (e.g., copying paper, gloves, tape, etc) in support of the <br />exercise? If so, receipts and proof of payment shall be included. <br />Canceled check, bank statement or transaction history (Showing the transaction was <br />processed by the bank) <br />Electronic Funds Transfer (EFT) Confirmation <br />Credit Card Statement & payment to credit card company for that statement <br />6. Have any expenditures occurred on rental of space/locations for exercises planning and conduct, <br />exercise signs, badges, etc.?
