(Consultant) CDM Project No. <br />I. PROTECT DESCRIPTION <br />Consulting Engineering services related to the preparation of a biennial water quality <br />report covering the last two years (1999-2000) of landfill water quality data. The report <br />is to be submitted to the Florida Department of Environmental Protection (FDEP), as <br />required by Chapter 62-701 of the Florida Administrative Code and the SWDD's FDEP- <br />issued landfill operation permit. <br />II. SCOPE OF SERVICES <br />Reference is made to Attachment B, the attached Scope of Services, and the <br />"Professional Engineering Services Master Agreement for Solid Waste." <br />in. CONSULTING ENGINEER INSURANCE REQUIREMENTS <br />The following insurance requirements will be provided in accordance with the Master <br />Services Agreement: <br />A. Worker's Compensation Insurance in accordance with Florida Statutes. <br />B. Automotive Liability with a minimum coverage of $1,000,000 for a combined single <br />limit. <br />C.
