40 <br />C_1 <br />M ED <br />11.6. The Bid shall contain an acknowledgment of receipt of all Addenda (the numbers of <br />which shall be filled in on the Bid Form). <br />11.7. The address to which communications regarding the Bid are to be directed must be <br />shown. <br />11.8. All supporting information requested in the Bid Form must be furnished. Do not <br />leave any questions or requests unanswered. <br />f i <br />12. Submission of Bids <br />12.1 Bids shall be submitted at the time and place indicated in the Invitation to Bid and <br />shall be included in an opaque sealed envelope, marked with the Project title and <br />name and address of the Bidder and accompanied by a completed Section 00411, <br />Statement on Public Entity Crimes, a completed Section 00412, Sworn Statement <br />Under the Florida Trench Safety Act the Bid Security and other required documents <br />which are included in the Contract Documents. If the Bid is sent through the mail or <br />other delivery system the sealed envelope shall be enclosed in a separate envelope <br />with the notation "BID ENCLOSED" on the face thereof. <br />12.2 The successful Bidder will be responsible for compliance with all applicable safety- <br />related Federal and State statutes and regulations, including, but not limited to, the <br />OSHA excavation safety standards, 20 C.F.R., Part 1926, Subpart P -Excavations, <br />k Trenching and Shoring, which are adopted by reference of the newly -enacted Florida <br />"Trench Safety Act", Section 553.60 et. seq., Florida Statutes. The Bidder who will <br />perform trench excavation work on a construction project with trench excavation(s) <br />over 5 feet in depth must allocate and include in its bid the cost of compliance with <br />trench safety standards and any special shoring requirements. The Bidder must <br />indicate in the sworn statement the method(s) he intends to use to achieve <br />compliance with each item (i.e., trench box, sheet piling, cribbing, wood shoring, or <br />other method selected by Bidder). <br />t <br />13. Disqualification of Bidders <br />13.1 One Proposal: Only one Proposal from an individual firm, partnership or corporation <br />under the same or under different names will be considered. If it is believed that a <br />Bidder is interested in more than one proposal for the work involved, all Proposals <br />in which such a Bidder is interested will be rejected. <br />13.2 Collusion Among Bidders: If there is evidence that collusion exists among the <br />Bidders, the Proposals of all participants in such collusion will be rejected, and no <br />participants in such collusion will be considered in future Proposals for the same <br />work. <br />INSTRUCTION TO BIDDERS <br />IB -5 <br />
