BUTLER, CLERK. <br />FY 2025 EMPA AGREEMENT <br />ATTACHMENT A (3) — QUARTERLY REPORTS <br />Recipients must provide the Division with quarterly financial reports and a final close-out report. <br />• Quarterly financial reports are due to the Division no later than forty-five (45) days after the end of <br />each quarter of the program year and must continue to be submitted each quarter until <br />submission of the final close-out report. The ending.dates for each quarter of this program year <br />are September 30, December 31, March 31, and June 30. <br />Reporting Period <br />Report due to Division no later than <br />July 1 through September 30 <br />November 15 <br />October 1 through December 31 <br />February 15 <br />January 1 through March 31 <br />May 15 <br />Aril 1 through June. 30 <br />August 15 <br />The Recipient shall provide all support documentation for the quarterly financial reports. <br />A. The Recipient must provide all supporting documentation for the quarterly financial reports. The <br />Division shall accept back up documentation by email if the County is not able to upload on <br />Salesforce. <br />B. The Quarterly Tasks form 1 B is due with your quarterly financial report each quarter. This form <br />identifies all Emergency Management personnel's required training completed (or working <br />towards completion) as well as quarterly deliverables during the agreement period. <br />C. In order to ensure compliance with Rule 27P-19.011, Florida Administrative Code, the Local <br />Budget Match Requirement Form shall be completed and sent when the Local County Budget is <br />approved or -by November.,15, 2025. The County shall provide a copy of the current Emergency <br />Management Local Budget (General Revenue) including approved budget date with the form. If <br />the County's current budget is lower than the previous year, or the average of the last three <br />years, the county is required to request a Waiver no later than forty-five (45) days after the county <br />budget is approved. <br />D. In a format provided by the Division, Form 4 — Staffing Detail and position descriptions of each <br />funded county emergency management staff shall be submitted no later than November 15, <br />2025, or along with 1St quarter reimbursement submission, whichever occurs first. <br />E. The final close-out report is due sixty (60) days after termination of this Agreement by August 30, <br />2026, or 60 days after completion of activities contained in this agreement, whichever occurs first. <br />F. An administrative closeout may be conducted when a recipient is not responsive to the Division's <br />reasonable efforts to collect required reports, forms, or other documentation needed to complete <br />the standard award and/or closeout process. FDEM will make three written attempts to collect the <br />required information before initiating an administrative closeout. If an award is administratively <br />closed, FDEM may decide to impose remedies for noncompliance per 2 C.F.R. § 200.339, <br />consider this information in reviewing future award applications, or apply special conditions to <br />existing or future award. <br />42 <br />
