Work Order No. 25 <br />43rd Avenue Pedestrian & Bicycle <br />Improvement Project PH II <br />be included on the Plan or summarized in an alignment table. <br />5. Plans shall include spot grades adequate to describe any proposed grading. Spot <br />grades will be provided to delineate ADA regulation compliance. <br />6. Match lines shall not be located within the limits of an intersection. <br />7. All quantities shall reference FDOT Pay Item Numbers. <br />8. All details shall reference FDOT Index Numbers. <br />9. All specifications shall reference to FDOT Specifications for Road and Bridge <br />Construction. Any deviations are special specifications not included in FDOT <br />Specifications are required in the Technical Specifications. <br />10. Preliminary (60% completion), interim (90% completion) and Final Plan (100% <br />completion) Submittal shall include the following: <br />i. One (1) set of drawings in PDF format. <br />ii. One (1) Opinion of Probable Construction Cost <br />The plans will be prepared based upon English units. Final design files will be submitted in <br />AutoCAD format. <br />D. Signing and Pavement Marking Plans: <br />Signing and Pavement Markings will be included within the Roadway Plan set. Quantities will <br />be provided in the Summary of Pay Items sheet. All signing and pavement markings are to be <br />prepared in accordance with the latest design standards and practices (MUTCD), FDOT <br />Standard Specifications, Indexes, and shall be accurate, legible, and complete in design. <br />E. Type I Categorical Exclusion: <br />The Consultant will attend one (1) kickoff meeting with the County to discuss the project <br />details. The Consultant will review readily available GIS databases to determine listed species <br />known in the area or those species that could potentially occur within the project corridor or <br />immediate vicinity. This will also include a review of the existing land use/land cover as mapped <br />by the St Johns River Water Management District (SJRWMD) and the existing soil survey. The <br />Consultant will also request information from the Florida Natural Areas Inventory (FNAI) and <br />the US Fish and Wildlife Service (USFWS) IPaC Trust Resources Report. Lastly, the Florida <br />Department of Environmental Protection (FDEP) OCULUS database will be researched for <br />existing contamination data within and adjacent to the project corridor. Following this desktop <br />review, the Consultant will conduct field reconnaissance to review the project corridor. <br />Representative photographs will be taken, and a photographic log will be prepared. <br />The Consultant will complete a Type I Categorical Exclusion checklist along with the <br />Page 4 of 8 <br />
