M J MASTELLER & MOLER, INC. <br />1655 27`h <br />, Ste. 2, Vero Beach, FL 32960 <br />M —CIVIL ENGINEERS — (772) 567-5300 <br />AMENDMENT 4 TO CCNA2018 WORK ORDER 1 <br />HOBART PARK BASEBALL FIELDS - IRC -1759 <br />EXHIBIT A <br />Construction of the Hobart Baseball Complex is underway. It has been discovered several existing <br />trees were found to be in a slightly different location than depicted on the site survey. As a result, <br />several of the trees need to be removed as they will be in conflict with the site's proposed <br />baseball field improvements. This Addendum is intended to provide landscape design & <br />permitting services to modify the past design to suit existing conditions. <br />In order to allow for the Hobart Park Baseball Complex improvements to be continued to be <br />constructed, we shall perform the following scope of services: <br />Scope of Work: <br />Task A — Update Landscape Construction Plans: As requested by the Parks, Recreation & <br />Conservation Department, we will visit the project site to confirm there is a need for certain <br />additional trees to be removed from the site due to their locations being in conflict with proposed <br />improvements. Once the site visits have been completed, we shall have the project landscape <br />architect, Sartain & Associates, prepare a revised landscape plan to reflect removal of additional <br />trees from the project site, which trees will need to be replaced by new trees, as applicable, in <br />order to confirm the site's overall landscape design shall remain in compliance with County code. <br />Task B — Approvals / Permits: Per Indian River County Planning & Development Services <br />Department, an Administrative Approval (AA) application needs to be applied for with $400.00 <br />review fee. <br />We will prepare the Administrative Approval application form for execution by the County Parks, <br />Recreation & Conservation Department. Once executed, we will submit the form, revised <br />landscape plan, cover letter and fee to the County Planning & Development Services Department <br />for approval. <br />The $400.00 review fee is included. No other permit review fees are included within our fees. <br />Task C — Responses to Comments: Following submittal of the AA application, we will respond to <br />reasonable comments (if any) pertaining to the proposed landscape modifications to assist to <br />secure a favorable AA process. <br />Task D— Notice of Plan Change: Following the successful AA process, we shall prepare a Notice <br />of Plan Change to be provided to the project Contractor such that the Contractor may construct <br />the landscape improvements in conformance with the approved revised design. <br />Page 1 of 5 <br />
