COMMUNITY HEALTH SURVEY — INDIAN RIVER <br />COMMUNITY HEALTH NETWORK <br />The Board reviewed the following Memorandum: <br />PUBLIC SERVICE ANNOUNCEMENT <br />FOR IMMEDIATE RELEASE <br />PLEASE CONTACT: GERRY KOZIEL, 567-4311, EXT. 1603 <br />(Vero Beach) The Indian River Community Health Network (IRCHN) is asking <br />for the cooperation of south county residents in a community health survey. The purpose <br />of the survey is to determine and prioritize the health needs of south county residents. <br />Surveys will be mailed to all households -with zip codes 32962 and 32968 and they <br />should be returned by May 19 in the envelope provided. <br />This information gathered in the survey will be used to prepare a report which <br />summarizes the information, prioritizes target populations, and identifies prevention, <br />education, and overall health care needs. This information will enable the IRCHN to <br />prepare a meaningful recommendation regarding the most appropriate, cost efficient <br />means of providing health care services including prevention and education to the <br />residents in the south county. The survey is completely confidential. <br />For more information, please call Gerry Koziel at 567-4311, ext. 1603. <br />South County Community Health Survey <br />Fact Sheet <br />WHO IS CONDUCTING THE SURVEY? <br />The survey is sponsored by the Indian River Community Health Network (see other Fact <br />Sheet) and funded by the Indian River Hospital District. <br />WHAT IS THE SURVEY? <br />The purpose of the survey is to determine and prioritize the health needs of south county <br />residents. The survey asks questions for demographic information, such as family size, <br />age, and sex. It also asks many health related questions, such as whether anyone in the <br />household has diabetes or heart disease and whether the family has a doctor. <br />HOW LONG IS THE SURVEY? <br />Five pages. <br />WHAT WILL BE DONE WITH THE INFORMATION FROM THE SURVEY? <br />The information will be used to prepare a report which will summarize the information, <br />prioritize the target populations according to prevalence, and identify prevention, <br />education, and overall health care needs. This information will enable the IRCHN to <br />prepare a meaningful recommendation regarding the most appropriate, cost efficient <br />means of providing health care services including prevention and education to the <br />residents in the south county. <br />MAY 6. 1997 112 <br />
