RECONDUNDATION: <br />I <br />Based on the totality of the information received and developed during the course of this <br />investigation, the EMS Director recommends the approval and issuance of a Class "C" <br />Certificate of Public Convenience and Necessity for Coastal Health Systems of Brevard, Inc., <br />limited to providing non -emergency interfacility neonatal transport service in Indian River <br />County for a period of two years, as provided for in Chapter 304 of the Indian River County <br />Cade of Laws and Ordinances. <br />ATTACE3EENTS: <br />1. Application For a Class "C" Certificate of Public Convenience and Necessity from <br />Coastal Health Systems of Brevard, Inc., with supporting documents. <br />21 Copy of letter from the Internal Revenue Service confirming tax exempt status of Coastal <br />Health Systems of Brevard, Inc. <br />3. Copy of letter from Dr. Javier Diaz -Blanco, Director of Neonatology Services at <br />Wuesthoff Hospital. <br />4. Copy of list of all current certificate holders in Indian River County with expiration <br />dates. <br />5. Photographs (5) of neonatal isolette and loading ramps. <br />6. Press Journal Confirmation of Public Hearing Notice. <br />The Chairman opened the public hearing and asked if anyone wished to be heard in <br />this matter. There were none. The Chairman closed the public hearing. <br />ON MOTION by Commissioner Adams, SECONDED BY <br />Commissioner Ginn, the Board unanimously approved for <br />issuance a Class "C" Certificate of Public Convenience and <br />Necessity for Coastal Health Systems of Brevard, Inc., limited <br />to providing non -emergency interfacility neonatal transport <br />service in Indian River County for a period of 2 years, as <br />provided for in Chapter 304 of the Code, as recommended in the <br />Memorandum. <br />OCTOBER 14, 1997 37 <br />BOOK 103 PACE 4 <br />
