UW -98 -02 -DS <br />AND US -98 -03 -CS <br />CHANGE ORDER NO.2 AND FINAL PAYMENT <br />BACKGROUND <br />On May 12, 1998, the Board of County Commissioners (BCC) approved a labor contract with <br />Driveways, Inc. in the amount of $48,040.54 and approved purchase of material in the amount of <br />$17,623.62 for a total construction cost of $65,664.16 to relocate miscellaneous water and force <br />mains in conflict with a proposed Florida Department of Transportation (DOT) drainage project in <br />the Rockridge subdivision (see attached minutes of meeting). On October 20, 1998, the BCC <br />approved Change Order No. 1, which adjusted the total labor contract amount to $74,057.04 and <br />the total material cost to $19,502.41 (see attached minutes of meeting), for a revised total <br />construction cost of $93,559.45. <br />Construction of the project is now complete. During construction, the following additional changes <br />were encountered: <br />• Wastewater operations requested the contractor to install two additional isolation valves on the <br />sewer collector main to minimise the number of customers affected by future outages. An Insta- <br />Valve was installed at one location under pressure, due to the number of customers impacted by <br />removing the main from service. A regular isolation valve was install at the other location, <br />since the affected section of force main had to be relocated and hence, removed from service. <br />• The contractor discovered that the small diameter force main that had to be relocated was a <br />different type of PVC material than originally anticipated. Fittings for the antiquated pipe are no <br />longer available. Consequently, the relocated pipe had to be constructed using ductile iron pipe, <br />since associated fittings are adaptable to the existing PVC pipe. <br />• Two of the water main conflicts identified in Change Order No. 1 were not needed, and <br />eliminated from the contract. <br />• A section of pipe that had to be replaced was discovered to be asbestos cement (AC) material. <br />AC pipe has been identified as hazardous material by the Occupational Safety and Health <br />Administration (OSHA), and is required to be; handled by certified contractors. Since <br />Driveways, Inc. is not qualified to handle AC pipe, the _health department had to oversee <br />packaging of the material. This caused a delay in construction until accommodations could be <br />made to have the hazardous material properly sealed and hauled to the landfill. <br />We are now requesting approval of Change Order No. 2, and final payment to the contractor. <br />FEBRUARY 2, 1999 <br />E <br />-41- BOOK PAGE 265 <br />
