EXHIBIT TWO: PROJECT PRIORITY # I <br />SECTION A. <br />PROJECT NARRATIVE - Family OPTIONS <br />Transitional Housing Program for Families with Children <br />1.Project Summary <br />The Options Transitional Housing Program will provide transitional housing and <br />supportive services for a period of six months to two years for (12) twelve homerless <br />families with children, at one time. The programs will provide homeless fim3 ies with <br />children the skills and supports they need to acquire and maintain safe, decent affordable <br />Permanent housing. By providing a stable living environment and appropriate supportive <br />services, the Family Options Transitional Housing Program will assure the homeless <br />fanmiVs progress toward achieving fiimily stability, financial independence and community <br />re -integration into safe decent permanent affordable housing. The Family Options Program <br />will fill a major gap the Continuum of Care, Le., provide transitional }musing with <br />supportive services for homeless fiamilies with children. According to the Continuum of <br />Care Gaps Analysis Chart, Samaritan Center is the only transitional housing program in all <br />of Department of Children and Families, District Fifteen. There are 9 family size rooms, <br />with up to 26 transitional housing beds for persons in families with children. These beds are <br />highly restricted to families who can me=et the eligibility criteria. There are no other <br />transitional housing beds of any kind in District Fifteen. The Continuum of Care has <br />identified transitional housing for families with children as the most critical need and <br />therefore the number one priority project. <br />Fiords are requested as follows: $230,000 for rehabilitation of an existing Adult <br />Congregate Living Facility (group homer) with the capacity to serve at least twelve (12) <br />families at one time, $58,000 for supportive services to families in transitional housing <br />including one year of follow-up after graduation by a Housing Specialist, and $15,000 for <br />operations. A photograph of the structure is attached. <br />The Indian River County Board of County Commissioners, as the applicant, is <br />requesting 5303,000 in Supportive Housing Program (SHP) funds to: (1) partially <br />fund the rehabilitation of an existing structure and (2) to assist in paying for the <br />required supportive services provided for homeless families with children by the <br />proposed Family OPTIONS Transitional Housing Program. <br />MAY 239 2000 <br />-65- BOOK PAGE <br />
