❑ (d) Name, certificate of authorization number, and street and mailing address <br />of the business entity on the map and report. <br />❑ (e) Name and license number of the surveyor and mapper in responsible <br />charge. <br />❑ (f) Name, license number, and street and mailing address of a surveyor and <br />mapper practicing independent of any business entity on the map and report. <br />❑ (g) Survey date (date of data acquisition.) <br />❑ (h) Revision date for any graphic revisions (when survey date does not <br />change.) <br />❑ (i) Map and report statement "Survey map and report or the copies thereof <br />are not valid without the signature and the original raised seal of a Florida <br />licensed surveyor and mapper." <br />❑ (j) Insurance statement in 1/4" high letters "The survey depicted here is not <br />covered by professional liability insurance" if there is no professional <br />liability insurance. <br />❑ (k) Additions or deletions to survey maps or reports by other than the <br />signing party or parties is prohibited without written consent of the signing <br />party or parties. <br />❑ (1) Ail computed data or plotted features shown on survey maps supported <br />by accurate survey measurements unless clearly stated otherwise. <br />❑ (m) Bearings, distances, coordinates, and elevations shown on a survey map <br />shall be substantiated by survey measurements unless clearly stated <br />otherwise. <br />❑ (n) Bearing reference (well established and monumented line) <br />❑ (o) A designated "north arrow" <br />❑ (p) Stated scale or graphic scale <br />❑ (q) Abbreviations in legend or notes. <br />❑ (r) Special conditions and any necessary deviation from the standards noted <br />upon the map or report. <br />❑ (s) Responsibility for all mapped features stated on the map or report <br />❑ (t) Map or report clearly states the individual primarily responsible for the <br />map or report when mapped features have been integrated with others. <br />(u) Map Accuracy. <br />(1) Vertical Feature Accuracy: <br />❑ (a) Vertical Control: Field -measured control for elevation information shown <br />upon survey maps or reports shall be based on a level loop or closure to a <br />second benchmark. <br />❑ (b) Closure in feet must be accurate to a standard of plus or minus .05 ft. <br />times the square root of the distance in miles. <br />❑ (c) All surveys and maps or reports with elevation data shall indicate the <br />01025-20 Technical Specifications <br />F:\Public Works\ENGINEERING DIVISION PROJECTS \1506 -Gifford Neighborhood 45th St Beautification\Admin\bid documents\Master <br />Contract Documents\Technical Specifications.doc <br />
