Mora, P.E., Public Works Director <br /> FROM: Richard B.Szpyrka, P.E.,County Engineer( <br /> SUBJECT: Work Order No. 2 Release of Retainage - Bridge Design Associates, Inc. for 66th <br /> Avenue Bridge Replacements for 41st Street,45th Street,49th Street,33rd Street and <br /> 37th Street Bridges IRC Project No.0545 <br /> DATE: June 2,2015 <br /> DESCRIPTION AND CONDITIONS <br /> On November 6, 2012, the Board of County Commissioners approved Work Order No. 2 with Bridge <br /> Design Associates, Inc. for services for the bridges below for the 66th Avenue (SR60 to 49th Street) <br /> project for a lump sum amount of$102,745.00: <br /> 66th Avenue Bridge over 41st Street—Professional Structural Engineering Services <br /> 66th Avenue Bridge over 45th Street—Professional Structural Engineering Services <br /> 66th Avenue Bridge over 49th Street—Professional Structural Engineering Services <br /> 66th Avenue Bridge over 33rd Street—Construction Assistance Services <br /> 66th Avenue Bridge over 37th Street—Construction Assistance Services <br /> 26th Street Headwall—Construction Assistance Services <br /> The as-built task provided for in Work Order No. 2 for the 66th Avenue Bridge over 41st Street was <br /> not required resulting in a savings of $1,730.00 thereby reducing the final contact total to <br /> $101,015.00. The Florida Department of Transportation has inspected and approved each <br /> component and Bridge Design Associates, Inc. has submitted Invoice No. 05-619/Ret dated <br /> 3/18/2015 for release of retainage in the amount of$10,101.50.
