BOARD OF COUNTY COMMISSIONERS <br />August 18, 2015 via Email <br />Kerns Construction & Property Management, Inc. <br />Attn: Mr. Daniel L Muschwec <br />1217 Delaware Avenue <br />Fort Pierce, FL 34950 <br />NOTICE OF AWARD <br />Reference: Indian River County Bid No. 2015036 <br />87th Street and 915t Avenue Sidewalk Improvements (Project No. 1205) <br />Dear Mr. Muschweck: <br />It is my pleasure to inform you that on August 18, 2015, the Board of County Commissionersawarded the above - <br />referenced project to your company. The following documents are required before the applicable County <br />department can issue a "Notice to Proceed" letter <br />1 Public Construction Bond (unnecorded)inthe amount of1OQ96ofthe contract amount ($1,084,854l). <br />Z. Two Signed Copies of Enclosed Agreement. <br />3. Certificate of Insurance indicating coverage required by Article 5 of the General Conditions (section <br />00700 of the bid documents) and Supplemental Conditions (Section 00800 of the bid documents). <br />Certificate(s) must name Indian River County as additional insured and must provide for a 30 day Notice <br />of Cancellation <br />In accorciance with section 255.05(1)(a[ Florida Statutes, you are required to execute a Public Consructinn Bond <br />for the above referenced project. Please submit the Bond, the Certificate(s) of Insurance and two fully -executed <br />copies of the enclosed agreement to this office at the address provided below no later than Wednesday, <br />September 2, 2015.
