Modular, 1893 90th Avenue <br />Design and Preconstruction <br />Emergency Services District <br />AMENDED WORK ORDER NO. 1 <br />FOR <br />DESIGN/BUILD AGREEMENT <br />PHASE IV <br />WITH <br />Barth Construction, Inc <br />This Work Order No 1 is in accordance with the existing DESIGN/BUILD AGREEMENT <br />PHASE I dated December 6, 2012 between Barth Construction, Inc (Design/Builder) and Indian <br />River County (County) <br />SECTION I — PROJECT LIMITS <br />This Work Order No. 1 outlines for the Design/Builder to perform in connection with design, and <br />preconstruction work and related design for a temporary modular building <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between Design/Builder and County, that the Design/Builder shall provide <br />design services to obtain and Indian River County Temporary Use Permit. Additional services <br />include; <br />• Site preparation and pad construction for modular delivery <br />• Civil design and permitting. <br />• Soil borings and reports as required <br />The proposal from the Design/Builder is attached to this work order and incorporated herein <br />1 <br />
