Indian River County <br />Emergency Services District Board of Commissioners <br />4225 43rd Avenue <br />Vero Beach, Florida 32960 <br />Telephone: (772) 226-3900 FAX: 772-567-9323 <br />Project: <br />Fire Station 1, 1500 Old Dixie <br />Site Work, Preparation and Improvements <br />Emergency Services District <br />WORK ORDER NO. 2 <br />FOR <br />DESIGN/BUILD AGREEMENT <br />PHASE IV <br />WITH <br />Barth Construction, Inc. <br />This Work Order No. 2 adds additional scope of work to Work Order No. 1 -dated December 16, <br />2014, is in accordance with the existing DESIGN/BUILD AGREEMENT PHASE I dated <br />December 6, 2012 between Barth Construction, Inc (Design/Builder) and Indian River County <br />(County) Work Order No. 2 agreement is as follows <br />SECTION I — PROJECT LIMITS <br />-4 <br />This Work Order No 2 includes tasks and services for the Design/Builder to perform in <br />connection with demolition and construction of the existing building. <br />SECTION II - SCOPE OF SERVICES <br />As agreed upon between Design/Builder and County, that the Design/Builder shall provide all <br />engineering, architectural and construction services to complete all tasks as outlined in Work <br />Order No. 2 specifically detailed in the attached proposal. The additional services include, <br />• Demolition of existing interior finishes on both the living and working sides of the station <br />building <br />• Minor structural enhancements to bring the structure into compliance with current <br />building codes. <br />• Two new storefront entrance doors and one new exterior window. <br />• Three new HVAC systems including new metal ductwork. <br />• New interior partitions, walls, floor and ceiling finishes <br />• New interior layout, finishes and equipment will generally match those provided and <br />installed at the recently completed Station 13 <br />1 <br />
