Frame, General Services Director -----&-A1-44 ZL1 <br />Subject: Change Order # 4, North County Library/Chilberg Construction Co., Inc. <br />BACKGROUND: <br />Attached is a memorandum from Lynn Williams regarding Change Order # 4. This change order <br />is the result principally of complying with requests from the Fire Inspector and requirements of <br />the Fire Code. Certain elements of this change order are staff related and are value added. This <br />is the last change order for the North County Construction Contract and its completion wraps up <br />outstanding work that needed completion <br />Change Order # 4 is composed of $15,609.40 of additional improvements. The approval of this <br />change order will increase the contract amount to $1,315,773.77. This increase represents a 6 % <br />increase in the construction bid amount. <br />Substantial Completion was issued on May 8, 2002 for this project. <br />RECOMMENDATION: <br />In light of the needed reasons identified for these changes, Change Order # 4 has been reviewed <br />and recommended by the supervising architect, and staff recommends approval of Change Order <br />#4 <br />ON MOTION by Commissioner Adams, <br />SECONDED by Commissioner Ginn, the Board, by <br />a 4-0 vote (Commissioner Macht absent) <br />unanimously approved Change Order #4, as <br />recommended by staff. <br />CHANGE ORDER IS ON FILE IN THE OFFICE <br />OF THE CLERK TO THE BOARD <br />JULY 2, 2002 <br />5 <br />-76- <br />
