Frame, General Services Director —Il.4-01/1-6114) <br />Change Order # 1 Dental Clinic/Health Department/Barth Construction, Inc. <br />BACKGROUND: <br />Attached is a memorandum from Lynn Williams regarding Change Order # 1 filed by Barth <br />Construction, Inc. The change order is for several items and includes both changes that increase <br />the contract cost and changes that decrease the cost. The net effect, however, is an increase of <br />$5,270.00 increasing the contract from $341,000.00 to $346,270.00. <br />Mr. Williams' memorandum identifies the items and briefly explains the reason for the specific <br />change. Some are staff requested and others were unforeseen items typically associated with <br />remodeling type projects Architect John Binkley has also reviewed the change order request. <br />Both Mr. Williams and Mr. Binkley have recommended approval of the requested change order. <br />Please note that proposal requests # 10 through # 13 are items specifically requested by the <br />Health Department and will be reimbursed to the County by the Health Department. These <br />proposal requests total $2,483 00 and reduce the net amount to be shared by the Board to <br />$2,787.00. The additional funding is available from the reserve fund for contingencies for the <br />Local Option Sales Tax. <br />RECOMMENDATION: <br />In light of the needed reasons identified for these changes, Change Order # 1 has been reviewed <br />and reconunended for payment by the supervising architect, and staff recommends approval of <br />Change Order #.1. Staff further recommends authorization for said payment to be made from the <br />reserve fund for contingencies for the Local Option Sales Tax. <br />ON MOTION by Commissioner Adams, SECONDED BY <br />Commissioner Ginn, the Board unanimously approved Change <br />Order 111 with Barth Construction, Inc., and approved payment <br />from the reserve fund for contingencies for the Local Option <br />Sales Tax, as recommended in the memorandum. <br />August 13, 2002 <br />CHANGE ORDER IS ON FILE <br />IN THE OFFICE OF THE CLERK TO THE BOARD <br />46 <br />
